Progressive hair loss can gradually affect the frontal hairline, temples, and overall scalp coverage. When hair loss reaches Grade 3–4, careful hairline planning and strategic graft placement become important to achieve balanced, natural-looking restoration.

This case study follows the 7-month hair transplant journey of a 34-year-old patient who had been experiencing hair loss for approximately 5 years. After clinical assessment, his hair loss was classified as Grade 3–4 baldness.

A customized BIG-FUE hair transplant was planned using 4,746 grafts in a single-day session. Following the procedure, the patient was advised supportive post-treatment care, including PRP and prescribed medicines, along with long-term hair maintenance.

Patient Consent Notice

This case study should be published only with the patient’s informed consent for the use of his clinical photographs and treatment details.

Hair transplant results vary from person to person depending on baldness grade, donor-hair quality, hair characteristics, scalp condition, healing response, progression of native hair loss, and adherence to post-operative care. The outcome of one patient should not be considered a guarantee of identical results for others.

Patient Profile & Diagnosis

The patient was a 34-year-old male who had been experiencing progressive hair loss for around 5 years.

His clinical profile showed no significant medical history, personal habits, or previous hair transplant treatment history. On assessment, the pattern of hair loss was categorized as Grade 3–4 baldness.

The main objective was to restore a more balanced frontal appearance while improving overall coverage and preserving donor resources for the future.

Primary Hair Loss Concerns & Expectations

Over several years, the patient had noticed progressive thinning and recession that had started affecting the overall appearance of his hairline.

His main concerns included:

  • Hair loss progressing for approximately 5 years
  • Grade 3–4 baldness
  • Recession of the frontal hairline
  • Reduced density in the frontal and upper scalp
  • Increased scalp visibility
  • Desire for improved frontal coverage
  • Preference for a natural-looking hairline
  • Need for a long-term hair restoration plan

Rather than simply increasing the number of hairs, the treatment goal was to create a natural transition between transplanted and existing hair while maintaining appropriate density and hair direction.

Clinical Assessment & Hairline Designing

Before planning surgery, the scalp and donor area were assessed to determine the appropriate technique, graft requirement, and distribution strategy.

The clinical evaluation focused on the patient’s baldness pattern, donor availability, recipient-area requirement, hairline position, hair direction, and long-term progression of existing hair.

Based on the assessment, the patient was considered suitable for a BIG-FUE hair transplant, with approximately 4,746 grafts planned for restoration.

A customized hairline design was created according to the patient’s age, facial proportions, degree of recession, and existing hair pattern.

Personalized Treatment Plan

Considering the Grade 3–4 baldness pattern and the area requiring restoration, an individualized treatment plan was prepared.

Surgical Technique

BIG-FUE Hair Transplant

Planned Procedure

Grade of Baldness: Grade 3–4
Technique: BIG-FUE
Grafts: 4,746
Sessions Required: 1-day session
Post Care: PRP + prescribed medicines

The treatment strategy focused on restoring the frontal area, improving visible density, creating better hairline framing, and distributing grafts efficiently across the recipient area.

Surgical Procedure

The patient underwent the planned BIG-FUE hair transplant in a single surgical session.

A total of 4,746 grafts were used according to the case details provided.

During implantation, attention was given to graft distribution, natural hair direction, angle, spacing, and transition between transplanted and existing hair so that the restoration could develop gradually and blend naturally over time.

Post-Operative Care

Following the procedure, the patient was advised to follow a structured recovery protocol. This included prescribed post-transplant medicines, gentle head washing from approximately the seventh day, a healthy diet, temporary avoidance of strenuous exercise and excessive sunlight, and use of a loose-fitting cap outdoors when advised.

Proper post-operative care is particularly important during the first few weeks because the transplanted grafts are still settling and the scalp is recovering.

Long-Term Hair Maintenance Plan

After the initial healing period, attention shifts from surgical recovery to maintaining both transplanted and existing hair.

The patient was advised to maintain a healthy diet and lifestyle, continue supportive treatment as prescribed, attend PRP sessions when recommended, and use Minoxidil or other maintenance therapy only when advised by the treating doctor.

Regular follow-up is also important because existing non-transplanted hair can continue to thin over time.

Month-by-Month Hair Transplant Growth Timeline

Week 1–2: Initial Healing

During the first couple of weeks, the transplanted grafts begin settling while the donor and recipient areas gradually heal.

Mild redness, crusting, scalp sensitivity, or temporary swelling may occur during this early recovery stage. These changes generally reduce progressively as healing continues.

Month 1–3: Temporary Shedding Phase

During the first few months, some transplanted hair shafts may shed temporarily.

This phase can sometimes concern patients, but shedding of the visible hair shaft does not necessarily mean the follicle has been lost. The transplanted follicular units remain beneath the scalp and begin entering their next growth cycle.

During this stage, visible density may temporarily appear reduced before new hair starts emerging.

Month 2–5: Early New Hair Growth

Around the fourth and fifth months, early transplanted hairs may begin becoming more noticeable.

At this stage, the new hairs can initially appear thin, short, soft, or uneven. This is expected because transplanted hair generally matures gradually rather than appearing at full thickness immediately.

Month 6: Progressive Hair Growth

By approximately six months, a greater number of transplanted hairs may become visible.

The hairline usually begins taking a clearer shape, while early improvements in coverage and density become easier to appreciate. However, the hair is still developing and has not reached its final thickness.

Month 7: Developing Density & Hairline Maturity

At the 7-month follow-up stage, the patient is still within the progressive growth phase.

By this point, transplanted hairs generally continue gaining length and thickness, while the hairline becomes more defined. Coverage can also continue improving as additional transplanted follicles enter active growth.

Seven months should therefore be considered an intermediate result rather than the final outcome.

Further improvement may continue over the following months as the transplanted hair matures.

7-Month Hair Transplant Result

This case documents the 7-month journey of a 34-year-old patient with Grade 3–4 baldness who underwent a BIG-FUE hair transplant using 4,746 grafts.

The procedure was completed in a single-day session, followed by supportive post-operative care with PRP and prescribed medicines.

At seven months, the patient remains within the active maturation period. Transplanted hair can continue becoming thicker, longer, and more uniform during the later months of recovery.
