The patient, a 40-year-old male, had been experiencing progressive hair loss for approximately 6 years.
Over time, the hair loss became more noticeable across the frontal and mid-scalp regions, resulting in significant scalp visibility and reduced overall hair density.
During clinical examination, his hair loss pattern was assessed as Grade 4–5 baldness.
He had no significant medical history reported, and after evaluating his scalp and donor area, he was considered suitable for hair transplantation.
The patient visited Hairfree & Hairgrow Clinic looking for a long-term hair restoration solution that could improve his hairline and provide better overall scalp coverage.

During the first two weeks, the transplanted grafts gradually settled into the recipient area while the donor area began healing.
Mild redness, crusting, and temporary scabbing around the transplanted follicles may occur during this early period.
The patient followed the recommended washing and post-operative care instructions to protect the newly implanted grafts.
As healing progressed, the redness and scabbing gradually reduced.
During the first few months after transplantation, the transplanted hair entered the temporary shedding phase.
This is a commonly observed stage after a hair transplant. While the visible transplanted hair shafts may shed, the follicles remain beneath the scalp and gradually prepare for their next growth cycle.
The patient was advised to continue the recommended hair-maintenance treatment during this period.
Because this is still an early stage of recovery, visible density during the first three months does not represent the final hair transplant result
New transplanted hairs gradually began emerging during this period.
Initially, the new hair appeared thinner and finer, but the density and texture started improving progressively.

By the later follow-up period, the transplanted hair had become more noticeable, with continued improvement in thickness, length, and density.
At the patient’s 11-month follow-up, the overall change could be appreciated more clearly compared with his pre-transplant condition

The transplanted hairs had continued to mature throughout the 9–11 month period, creating better visual coverage across the treated areas.
At 11 months, the patient’s progress showed a significant improvement compared with his initial Grade 4–5 baldness presentation.
A total of 4,679 grafts were implanted using the BIG-FUE technique. The procedure was completed over a two-day surgical session because of the extent of baldness and the number of grafts required.
Early growth became visible between 4–6 months, while more significant improvement in density, hairline definition, and scalp coverage was seen during the 9–11 month period.
BIG-FUE may be suitable for patients with Grade 4–5 baldness when the donor area has adequate graft availability and hair quality. In this case, the donor hair quality was assessed as good, and the treatment plan focused on reconstructing the frontal hairline and improving mid-scalp coverage.
